Reproduction factory inspection card for Bally games. Used on many games over the decades including electro-mechanical (EM) and electronic solid-state (SS) models. Commonly found in main cabinet, backbox, and on playfield under-side. Employees at the factory would sign off on this card (employee number) as inspections/tests were done on games coming down the assembly line. See additional image below for sample of filled-out & mounted card.
